Who is my insurance carrier / plan administrator?

OSB plans work a little differently than traditional insurance — here's who actually administers your coverage and which network to give your provider.

You may notice there's no traditional insurance company name (like Blue Cross, Aetna, or Cigna) on your plan. Here's why.

OSB plans are self-administered, not carrier-insured

Your plan is administered by Staff Benefits Management & Administrators (SBMA) and uses the PHCS by MultiPlan provider network for in-network care. There is no traditional insurance carrier acting as the underwriter.

What this means for you

  • If a provider's office asks "who's your insurance," tell them PHCS/MultiPlan — that's the network they'll recognize for verifying your coverage.

  • If you need to reference your plan administrator specifically, that's SBMA.

  • Your dental coverage is separately administered by Delta Dental PPO, and vision by VSP.
